A morning coffee may be doing more than clearing the fog before school or work. A large 13-year study links daily coffee drinking, including decaf, with lower risks of cirrhosis, the most common liver cancer, and death from liver disease.
The clearest benefits appeared around three to four cups a day, although lower risk was already visible at one to two cups. Still, this was an observational study, so it cannot prove that coffee prevented disease or that everyone should start drinking more.
A closer look at 354,957 adults
Transplant hepatologist Hyunseok Kim led the Cedars-Sinai Health Sciences University team that analyzed 354,957 UK Biobank participants who began without cirrhosis or hepatocellular carcinoma. Hepatocellular carcinoma is the most common primary liver cancer, meaning cancer that starts in the liver.
Participants reported how much coffee they drank, whether it was caffeinated or decaffeinated, and whether they added sugar or artificial sweetener. Researchers followed linked health records for a median of 13 years, then compared coffee habits with new diagnoses and liver-related deaths.
This was not only a paperwork exercise. The team also examined liver MRI scans from 28,961 people and blood protein profiles from 44,633, looking for signs of fat, iron buildup, inflammation, and scarring.
The risk numbers
Compared with non-drinkers, people having one to two cups a day had a 20 percent lower risk of cirrhosis, a 24 percent lower risk of liver cancer, and a 31 percent lower risk of liver-related death. That is a notable pattern for a drink poured before breakfast.
At three to four cups, the risks of cirrhosis and liver cancer were each 35 percent lower, while the risk of liver-related death was 41 percent lower. Overall, this range produced the most consistently favorable results.
People drinking five or more cups had a 32 percent lower risk of cirrhosis, a 47 percent lower risk of liver cancer, and a 42 percent lower risk of liver-related death. These are relative risks against non-drinkers, not a person’s absolute chance of illness.
What cirrhosis means
Cirrhosis is advanced, permanent scarring that replaces healthy liver tissue and makes the organ less able to do its job. It can develop after years of fatty liver disease, viral hepatitis, heavy alcohol use, or other long-term damage.
Why does that matter for cancer? Chronic liver disease and cirrhosis are major risk factors for hepatocellular carcinoma, although the cancer can sometimes develop without cirrhosis.

Decaf offers an important clue
The association looked similar among people drinking caffeinated and decaffeinated coffee. That suggests caffeine is not the whole explanation and points toward other natural compounds with antioxidant effects, although the exact ingredient remains unknown.
The MRI results added weight to that idea. Higher intake was linked with less liver fat, lower iron levels, and fewer signs of inflammation and early scarring, while blood tests showed a more favorable pattern of proteins tied to liver function.
This fits earlier evidence. A 2021 study using the same database associated instant, ground, and decaf coffee with fewer chronic liver disease outcomes, while a 2015 multiethnic cohort linked coffee with lower liver cancer rates and fewer deaths from chronic liver disease.
Sugar changes the picture
Adding sugar or artificial sweetener did not erase the lower rates of cirrhosis, liver cancer, or liver-related death. But users had 36 percent higher odds of an elevated MRI marker tied to liver inflammation and scarring.
In practical terms, coffee does not turn a syrup-heavy drink into a health food. The American Heart Association recommends keeping added sugar below about six teaspoons a day for most women and nine teaspoons for most men.
Coffee is not a liver treatment
The researchers adjusted for age, smoking, alcohol use, body weight, metabolic health, socioeconomic factors, and a genetic liver-risk marker. Even so, coffee drinkers may differ from non-drinkers in ways that questionnaires and statistics cannot fully capture.
Intake was self-reported, most participants were of European ancestry, and the imaging subgroup was healthier than the full group. Those limits matter. A randomized trial would be needed to show that coffee itself prevents cirrhosis or cancer.
A sensible daily approach
Senior author Ju Dong Yang said, “Our findings support moderate coffee consumption for people who already enjoy and tolerate it well.” The team did not advise non-coffee drinkers to begin solely for liver protection, and coffee cannot replace exercise, weight management, limiting alcohol, or medical care.
More is not automatically better. The FDA says the caffeine in roughly two to three 12-ounce cups is not generally associated with negative effects for most adults, although sensitivity varies widely.
Caffeine can also follow you into the night. A sleep study found that a substantial dose taken even six hours before bedtime could disrupt sleep, so an afternoon cutoff may help anyone staring at the ceiling after a late cup.
