Most people think of dirty air as a threat to the lungs and heart. A large Danish study published in May 2026 links years of exposure to common air pollutants with a higher risk of dementia with Lewy bodies and Parkinson’s disease dementia.
The pattern was strongest for fine-particle pollution, although the research does not prove that polluted air directly caused either condition.
Researchers studied 3,024 people with dementia with Lewy bodies and 3,808 with Parkinson’s disease dementia, along with more than 68,000 matched controls. People living in areas with higher levels of fine particles and nitrogen dioxide were consistently more likely to receive either diagnosis.
The findings put a new spotlight on something people breathe every day, usually without seeing it.
A decade of exposure
The team used Denmark’s nationwide health records to identify diagnoses made from 2001 through 2021. Each person with dementia was matched with 10 people of the same age and sex, and researchers estimated pollution around every residential address during the previous 10 years.
The analysis also considered income, education, employment, other illnesses, psychiatric history, and neighborhood conditions.
Dr. Dimitry S. Davydow of University of Florida Health led the work with researchers at Aarhus University. “These are pollutants most people are exposed to every day,” he said, pointing to traffic, shipping, and other forms of combustion as common sources.
The strongest results involved PM2.5, the name for extremely small airborne particles.

For each increase of 5 micrograms per cubic meter, the model linked PM2.5 with risk estimates 3.7 times as high for dementia with Lewy bodies and 2.4 times as high for Parkinson’s disease dementia. The unit used is the standard measure U.S. air-quality agencies use for tiny amounts of pollution.
What the pollutants are
PM2.5 is made up of particles so small that the largest are about 30 times narrower than a human hair. They can come from vehicle exhaust, industry, power generation, wood burning, and wildfires, or form when gases react in the air. You will not always see them, even when they are present.
Nitrogen dioxide is a gas produced mainly when fuels burn. Traffic jams bring the familiar noise and exhaust fumes, but they can also raise roadside levels of this pollutant.
For each increase of 10 micrograms per cubic meter, nitrogen dioxide was associated with a 95% higher risk estimate for dementia with Lewy bodies and a 14% higher estimate for Parkinson’s disease dementia.
Two related dementias
Lewy body dementia is an umbrella term for two closely related disorders. Both involve abnormal clumps of a protein called alpha-synuclein inside brain cells, and those deposits are known as Lewy bodies. They can disrupt thinking, attention, movement, sleep, mood, and behavior.
The timing of symptoms usually separates the diagnoses. In dementia with Lewy bodies, thinking problems appear before movement symptoms or within about a year of them.
In Parkinson’s disease dementia, cognitive decline begins after a person has lived with Parkinson’s movement problems for more than a year.
Why does that distinction matter? The two diseases share much of their biology, but they may not respond to environmental exposures in exactly the same way. In this study, the pollution link was consistently stronger for dementia with Lewy bodies.
How pollution may reach the brain
Scientists still do not know the exact biological route. One possibility is neuroinflammation, which means a long-lasting immune response inside the brain that may damage nerve cells over time.
Fine particles may also promote oxidative stress, a form of cellular wear caused when the body’s protective systems cannot keep up.
The Danish findings build on a 2025 Science study that combined U.S. health records with experiments in mice.

That work linked long-term PM2.5 exposure with hospital diagnoses involving Lewy body disease and found that exposed mice developed abnormal alpha-synuclein clumps, brain shrinkage, and cognitive problems.
The animal results offer a possible mechanism, but they still do not show exactly what happens in people.
What the study cannot prove
This was an observational study, so it found an association rather than cause and effect. Registry data may have missed milder cases, and the researchers could not fully measure workplace pollution or lifestyle factors such as smoking, diet, and exercise.
Fine particles and nitrogen dioxide also tend to rise together, making it difficult to separate their individual effects.
Still, the study had notable strengths. It drew from more than 2.1 million Danish residents ages 65 to 95, used detailed address histories, and followed exposure over a decade rather than taking a one-day snapshot. The results also remained after the team adjusted for a wide range of medical and social differences.
At the end of the day, cleaner air would not remove every cause of dementia. But because air pollution can be reduced through transportation, energy, and industrial policy, it may represent a potentially modifiable piece of a much larger brain-health puzzle.
