That basic link helps explain why milk thistle is marketed to people who feel heavy after fried food or a large dinner.

But a plausible mechanism is not the same as proof that a capsule or tea will stop gas or shrink a bloated abdomen.

What the review found

Caie Wang, a gastroenterology researcher at the General Hospital of Northern Theater Command in China, was the first author of the 2025 review.

The team analyzed 17 randomized studies involving 2,069 adults with metabolic dysfunction-associated steatotic liver disease, the newer name for a common fatty liver condition tied to metabolic problems. Nine trials tested silymarin alone and eight tested it in combination products.

Compared with no treatment or a placebo, silymarin alone may have reduced two liver enzymes, but the researchers were “very uncertain” about those results. Effects on another major liver enzyme and on other side effects remained unclear. These are laboratory markers, not direct proof of symptom relief or disease reversal.

Read More: Psychology says people who still write in paper calendars instead of their phone are not old-fashioned; they are doing something for their memory

The gaps matter. None of the trials reported results for deaths from any cause or health-related quality of life, most studies were small, and findings varied widely from one study to another. The review therefore concluded that the benefits and harms of silymarin for this liver condition remain unclear.

Bloating is a separate question

Bloating usually starts in the digestive tract, not in a blood test. Gas can build when people swallow air or when gut bacteria break down carbohydrates that were not fully digested. Constipation, lactose intolerance, and irritable bowel syndrome are also common reasons the waistband feels tighter after eating.

Milk thistle trials reviewed for fatty liver disease were not designed to test ordinary post-meal bloating. More awkward still, the National Center for Complementary and Integrative Health lists bloating, gas, and nausea among milk thistle’s most common side effects.

The supplement someone takes to feel less swollen could, in some cases, create the same complaint.

Other plant-based options have more direct evidence for certain gut problems. The American College of Gastroenterology conditionally suggests peppermint for overall irritable bowel syndrome symptoms, but it also labels the evidence low quality and notes that only a small number of commercial products have been rigorously tested.

That does not make peppermint a universal answer, but it shows why the cause of bloating matters.

A butterfly rests on a purple milk thistle flower, the plant whose seeds contain the compound silymarin.

Daily use is not simple

Tea, liquid extract, and capsules are not interchangeable. They can deliver very different amounts of silymarin, and commercial products do not always contain what the label suggests. In the United States, supplements are not approved by the Food and Drug Administration for effectiveness before they reach store shelves.

No evidence-based daily milk thistle dose has been established for preventing bloating.

Read More: A cardiologist and a dietitian agree on the #1 snack for your heart, and it is not the handful of nuts you were told to eat

Anyone considering regular use should review medications and allergies with a doctor or pharmacist, especially because the plant can trigger reactions in people sensitive to ragweed, daisies, chrysanthemums, or marigolds. Safety during pregnancy and breastfeeding is not well known.

Most of the heavy lifting for metabolic fatty liver still comes from habits, not herbs. For people with this condition who have excess weight, guidance from the American Association for the Study of Liver Diseases emphasizes a balanced eating plan, regular activity, and gradual weight loss.

Losing about five to ten percent of body weight is associated with improvements in liver fat, inflammation, and scarring.

When to seek medical care

Persistent bloating deserves attention when it is new, frequent, or getting worse. Severe pain, repeated vomiting, fever, yellow eyes or skin, dark urine, pale stools, unexplained weight loss, or rectal bleeding are warning signs that need medical evaluation.

Pain lasting several hours after a fatty meal can also point to a gallbladder problem rather than simple gas.

The bottom line is less dramatic than supplement ads. Milk thistle may influence some liver enzyme readings, but it has not been shown to reliably treat daily bloating or replace a diagnosis and proven care.

The main review was published in the Cochrane Database of Systematic Reviews .

That tight, swollen feeling after a rich meal can make a “liver support” supplement sound like an easy answer.
