A study of more than 164,000 people in the United Kingdom and United States suggests that accelerated biological aging may be one piece of the puzzle.

In simple terms, some younger bodies looked older on the inside than their birthdays would suggest.

People from more recent generations showed larger gaps between biological age and chronological age, and those gaps were associated with a higher risk of solid cancers diagnosed before age 55.

The findings do not prove that faster aging causes cancer, but they offer a possible bridge between lifelong exposures and disease appearing earlier.

Biological age is not your birthday

Chronological age is the number of years since someone was born. Biological age estimates how well the body is functioning by reading signs such as inflammation, immune health, metabolism, and accumulated cellular damage.

Read More: You’ve been told to eat more protein, but a nutritionist says that in most cases you don’t need to; here’s what actually happens

Think of it as the difference between a car’s model year and the condition under its hood. Standard deviation describes how far scores spread around an average, so this does not mean the younger group literally aged 23% faster.

Read More: John van Swieten, neurologist on hereditary dementia: ‘Most don’t want to know, precisely because there’s still no treatment’

The U.S. validation group showed the same general direction, with a larger age gap in the youngest birth group than in the oldest. Simply put, later generations tended to carry blood profiles that looked less healthy for their actual age.

Cancer risk rose with the age gap

For each one-step rise on the standardized PhenoAge scale, the risk of an early-onset solid cancer was 8% higher.

People in the most biologically aged third had a 15% higher risk than those in the least aged third, with the clearest associations involving lung, gastrointestinal, and uterine cancers. Solid cancers form in organs and tissues rather than in the blood.

The organ-level results added another clue. Faster immune-system aging was associated with lung cancer before 55, while older-looking fat tissue was linked to early colorectal cancer, which begins in the colon or rectum.

Early cancer is still uncommon

The concern comes from a real shift, not from cancer suddenly becoming common among young adults. An official UK analysis found that cancer rates among people ages 25 to 49 rose 24% from 1995 to 2019, although about nine in ten cancer cases still occur after age 50.

Colorectal cancer has become a particular focus. A separate global analysis reported rising rates among adults ages 25 to 49 in 27 of 50 countries and territories studied, even though only a small share of all colorectal cases occur before 50.

What might be wearing the body down

The study did not identify one culprit. Instead, biological age may work like a dashboard warning light, capturing the combined effect of diet, body weight, inactivity, alcohol, smoking, air pollution, and other social or environmental exposures without showing which one matters most.

Some of those pressures are woven into ordinary days, from fast food grabbed between shifts to long hours sitting and breathing traffic exhaust. But this research cannot say that any single habit or pollutant caused the age gaps or the cancers that followed.

Important limits remain

This was an observational study, so it found associations rather than cause and effect. The UK sample was 92% white, and the biological aging tools need wider testing in younger and more diverse populations.

Read More: If your blood sugar keeps spiking, these four simple snacks could help bring it down naturally, and you likely already have them

The definition of early-onset cancer matters, too. The main analysis counted diagnoses before age 55, though the overall pattern remained when researchers used an under-50 cutoff. The organ-specific findings came from a smaller subset and also need confirmation elsewhere.

What could happen next

The researchers say blood-based aging measures could eventually help identify younger adults who may benefit most from prevention or earlier testing. They are not ready to replace established cancer screening, and no one should read a biological age score as a diagnosis. For now, it is a clue.

The work forms part of Team PROSPECT, supported through Cancer Grand Challenges by Cancer Research UK and the U.S. National Cancer Institute. That group is studying how environmental, lifestyle, and social changes become embedded in the body over time.

The full study was published in Nature Medicine.

Why are more adults developing cancer before middle age?
